What we treat

We help adults recovering from illness, injury, or surgery—especially when leaving home takes considerable effort. Common reasons for PT at home include:

What a typical visit looks like

  1. Home Health PT Evaluation – We review your medical history, medications, current abilities, and goals, then create a personalized plan of care.
  2. Hands-on treatment – Guided exercises, mobility and gait training, pain and edema management, and education to support daily life.
  3. Home exercise plan – Easy-to-follow instructions with the right level of challenge.
  4. Progress updates – Regular communication with your physician and nursing/therapy team.

Who qualifies for in-home Physical Therapy?

  • Your physician orders home health services, and
  • You have a condition that requires skilled therapy, and
  • Leaving home requires considerable effort and occurs infrequently or for short durations.
    Many patients receive PT alongside Nursing, Occupational Therapy, Speech Therapy, Medical Social Services, and/or Home Health Aide support.
