Professional care, at home
Our Home Health services are administered in the patient’s home by professionally trained and highly experienced staff. These services are covered by Medicare. Medicare Advantage Plans, and certain commercial insurance plans. Your recovery will be achieved by intermittent home visits, the frequency of which will be determined by your physician and our assessing clinicians.

Who is eligible for home health services?
Eligible individuals are homebound and require the need for skilled nursing care and/or therapy services.
Homebound is defined by Medicare as “considerable and taxing effort to leave the home, infrequently, and for short durations of time.”
A physician must order home health services. We can assist you in obtaining an order from your current physician.
