What do medical social workers do?
- Each visit begins with a thorough social, financial, and environmental evaluation to understand the patient’s living conditions, support system, and potential barriers to recovery.
- From there, our social workers assist with short- and long-term care planning—clarifying goals, preparing for future needs, and ensuring that patients and families understand their options.
- We also coordinate essential community resources, such as transportation services, meal programs, support groups, financial assistance, and long-term placement when appropriate.
How we can help
Our role is to remove obstacles, stabilize the home environment, and connect families with the right tools so that care remains consistent, safe, and sustainable. Through close collaboration with the full Sunrise clinical team, our social workers ensure that each patient’s plan of care is comprehensive, realistic, and aligned with their health and personal priorities.
